According to what I've read, fat grafting is permanent (excluding the fact that you can lose weight naturally, which will affect fat grafts as well as "normal" fat). However, in order for a fat graft to survive, it needs to have a blood supply of it's own, meaning blood vessels have to grow. Usually about 40-70% of the fat survives, depending on the person. It's more likely to survive in areas that don't move as much (like cheeks and forehead) than in areas that do move a lot (like lips). Because there's a limit to how much fat you can graft at one time, fat grafting is done over multiple appointments to ensure that you get the amount of fat you want in your cheeks even after some of it is absorbed back into your body.

All that being said, there is always the risk of it all absorbing back into your body, and as you age the fat grafts may not look the same due to natural weight loss/gain and changes in your face (which is true for any cosmetic surgery). Neither fat grafting nor implants are guaranteed to work forever (implants can become infected, and may also not look the same due to weight or facial changes) so it comes down to which one you are comfortable with and will give you the results you want in the immediate future.
